Best practices to avoid fake profiles?
When offering to create a profile (for example, login+pwd) for a web service, what are the best practices one should implement to avoid mass spamming/creation of fake profiles? I am thinking about email confirmation, captchas, etc… any other ideas that work in practice?

I have a website e-mail form. I use a custom CAPTCHA to prevent spam from robots. Despite this, I still get spam.
Multiple tries with captcha causing error
At this particular website, I and several others, are having the same issue. That issue is that even though all images are checked, we are getting errors saying that we haven’t clicked on every picture available of whatever was needed to be checked. Sometimes you have to go through 10-20 tries to finally be successful. And when you have to try that many times, you then get the error that is in the image.
